Health and Immunization Records

Introduction

To assist in maintaining your health and the health of others on campus, Student Health Services offers a caring, confidential environment. The maintenance of a student health record is essential for all students. Please read below to understand the requirements for students living on and off campus.

The Student Health and Immunization Record must be submitted on or before August 1, 2025.  

The Healthy Stevens Portal can be found in the apps on MyStevens. Instructions on how to upload your record can be found here.

Welcome to the Stevens community!

Student Health Services assists in maintaining your health and the health of others on campus in a caring, confidential environment. The State of New Jersey requires each student attending state-accredited institutions of higher education to provide his/her complete Student Health and Immunization Record. This is due before you move into residence halls and/or before you start classes. Every undergraduate, graduate, international, domestic, full-time, part-time and transfer student on campus must have a completed Student Health and Immunization Record on file with Student Health Services. If you are taking all of your classes online, you are NOT required to submit a Student Health and Immunization Record. If you meet on campus for even one (1) class, then please follow the instructions for “Part-Time” students.  Please note the following requirements have been broken down into enrollment categories:

FULL-TIME Undergraduate Students (Domestic, International, Transfers)

To comply with New Jersey state law and Stevens' requirements, you must be immunized/tested for the certain illnesses prior to attending classes and must provide documented proof in English:

  1. MMR vaccination Required: (unless born prior to January 1, 1957)- Measles, Mumps, Rubella

    1. Two (2) doses of measles vaccine or blood titer laboratory proof of immunity

    2. Two (2) doses of mumps vaccine or blood titer laboratory proof of immunity

    3. Two (2) doses of rubella vaccine or blood titer laboratory proof of immunity

  2. Hepatitis B vaccinations Required (administered as a series of three (3) shots)
    The first two vaccinations should be one month apart, and the third vaccination should be at least six months after the first. We also accept proof of immunity if you have a copy of a blood titer test HbsAB, to verify immunity. The Hepatitis B Surface Antibody must be printed on the report as either a “+” or “positive”

  3. Varicella vaccination Required Two doses of the vaccine, documentation of previous infection, or varicella antibody titer (IgG).

  4. Meningococcal meningitis Required Both Meningococcal ACWY vaccination (booster dose required if last dose was given 5 years before the start of classes or before the 16th Birthday) AND Meningococcal B vaccination are Required.

  5. Covid Vaccination It is strongly recommended to keep up to date with current booster recommendations. All new students should submit any Covid vaccinations received. 

FOR INTERNATIONAL STUDENTS ONLY
Required:
Quantiferon Gold
 Interferon-based assay (blood test) or PPD  (skin test for Tuberculosis) no more than 6 months prior to the start of classes regardless of whether you received BCG vaccine as a child.
A chest x-ray (within the last two years) written report is acceptable as well (Do not send the actual x-rays).

PART-TIME/FULL-TIME Graduate Students (Domestic)

To comply with New Jersey state law and Stevens' requirements, you must be immunized/tested for the following prior to attending classes and must provide documented proof in English:

  1. MMR vaccination Required: (unless born prior to January 1, 1957)- Measles, Mumps, Rubella

    1. Two (2) doses of measles vaccine or blood titer laboratory proof of immunity

    2. Two (2) doses of mumps vaccine or blood titer laboratory proof of immunity

    3. Two (2) doses of rubella vaccine or blood titer laboratory proof of immunity

  2. Hepatitis B vaccinations Required (administered as a series of three (3) shots)

ONLINE/WEB CAMPUS Graduate Students

  • No Student Health and Immunization Record Required

Conclusion

It is very important that you complete the Student Health and Immunization Record in its entirety. An incomplete Record will result in a Registrar’s Hold being placed on your account. This means you will not be able to register in person or via the Web for any future classes until all forms are received by Student Health Services.
