Welcome to the Stevens Institute of Technology Report a Concern page. These forms are available to all students, faculty, staff, guests, and visitors to report incidents that occur within our community. Please note that this form is not intended for emergencies. To report an emergency, contact Campus Police immediately at 201-216-3911.
To begin, select the option that best describes your affiliation with Stevens:
Current Stevens Students, Faculty, and Staff
– Select this option if you have an active stevens.edu account and can log in using your Stevens credentials.
Individuals Without an Active Stevens Account
– Select this option if you do not have an active stevens.edu account.
After selecting the appropriate button below, you will be directed to the reporting form that best matches your access type and reporting needs.
Bias Incident
Report a concern regarding discrimination, harassment, and bias, including discrimination under Title VI. Title VI prohibits discrimination based on race, color, or national origin including harassment, based on their actual or perceived shared ancestry, ethnic characteristics, or citizenship/residency in a country with a dominant religion or distinct religious identity.
CARE Referral
The CARE team responds to student behavior that is concerning and provides support that a student needs including academic, mental health, medical, or other resources depending on the nature of the concern.
Please note that the CARE Team is not an emergency resource. For emergencies, call Stevens Police at 201.216.3911.
Campus Incident
Submit a report for an incident related to campus safety, disturbance, hazing-related incident, complaint, conflicts, etc.
For building or facilities issues, please submit a work order by visiting https://siot.webtma.com/ or by going to your myStevens dashboard and clicking on the "WebTMA7" "Facilities Work Order" icon. If you encounter any issues entering a Work Request, please contact [email protected].
Title IX
Report a Title IX concern.
Graduate Student Code of Academic Integrity Violation
Report a perceived violation of the Graduate Student Code of Academic Integrity to the Graduate Student Code of Academic Integrity Board.
Accessibility Concern
Submit a report for all access concerns.
Submit a report to the Stevens Honor Board for an incident related to an Honor System violation.
Honor System Violation
Submit a report to the Stevens Honor Board for an incident related to an Honor System violation.
Other Academic Concerns (NOT FOR ACADEMIC INTEGRITY CONCERNS)
Submit a report to a specific school or department about a course, curriculum, or instructor. This link should not be used to report Honor System or Graduate Student Code of Academic Integrity violations.
Stevens Compliance Hotline
The Hotline serves as a channel to report or seek guidance on possible compliance issues with the option to report anonymously. It is supported by a third-party vendor, Navex, and staffed by professionals. Phone: 1-855-277-4065
International, Study Abroad, Service Trip, and Away from Campus Report
Submit a report for an incident that has occurred away from Stevens campus, either internationally, study abroad, service trip, or any location away from campus
Parking or Shuttle Concerns
Submit a report for an incident related to Stevens parking or Stevens shuttle concerns
Uncategorized (Other)
Submit a report for all other concerns
Summer Semester Incident (June 1 - August 5)
Submit a report for an incident that occurred during a summer program at Stevens (June 1 - August 5)
