Health and Immunization Requirements

The Healthy Stevens Portal

The Healthy Stevens Portal provides student access to campus wellness offices and resources and serves as the primary point of communication for students regarding health and wellbeing. You should use the Healthy Stevens Portal to complete and upload all required health and immunization documents and/or vaccine exemption requests for their initial enrollment at Stevens.  Varsity athletes also submit and complete all required health information for Athletics through the Healthy Stevens Portal. Once you arrive on campus, you can use the Healthy Stevens Portalto make appointments at Student Health Services and Counseling and Psychological Services (CAPS). Access the Healthy Stevens Portal through myStevens .

Health and Immunization Records

In preparation for arrival on campus, all students are required by Stevens and the State of New Jersey to submit completed electronic health and immunization records by August 1. You should schedule an appointment for a physical with your primary care physician as far in advance as possible so that forms can be completed in a timely manner. All students, including those who choose to commute, must complete the Health and Immunization Forms. Varsity Intercollegiate Division III athletes at Stevens are required to complete supplemental Varsity Athletic Medical Forms, which are electronic and available on the Healthy Stevens Portal. Blank physical and immunization forms are provided on the portal to download to take to your healthcare provider. Health Insurance Requirement

In addition to providing your records to Student Health Services, Stevens requires all undergraduate students to have health insurance. All undergraduate students will be billed for the cost of the Stevens Student Health Insurance as they begin their first semester and for all subsequent fall semesters unless the student waives the insurance coverage annually. Students waiving Stevens’ Student Health Insurance must demonstrate proof of health insurance from another U.S.-based provider that meets federal requirements (see website linked below for details) before September 15. COVID-19 Vaccinations

Stevens strongly recommends students and employees stay up-to-date with Covid-19 boosters, both new and returning students coming to campus should upload any Covid-19 vaccinations through the Healthy Stevens portal.
