Special Course Forms Undergraduate Permission to Take Graduate Courses
This form is required of Stevens Undergraduate students taking all 600 level courses and above in the Charles V. Schaefer, Jr. School of Engineering and Arthur E. Imperatore, Sr. School of Sciences and Arts and all courses in the Wesley J. Howe School of Technology Management.
Form Permission To Take A Graduate Course Under The Pass-Fail Grading System
This form is required before you can take a course at Stevens with a Pass/Fail grade.
Form Incomplete/Absence Petition
If you cannot complete the requirements for a course and/or are absent for an final examination, you can have an extension of time granted to complete the requirements for the courses. A grade of INC or ABS will be recorded while requirements are being met.
Form Permission to Take Course at Another Institution
This form is required before you can take a course at another institution for the purpose of a transfer credit if you are currently enrolled as a Stevens graduate student. A Transfer Credit form needs to be submitted after completion of the course.
Form Transfer Credit
Official transcripts are required from the institution you are transferring credits from.
Form Grade Change
If you suspect that a grade has been incorrectly reported, see the instructor of the course and he will submit this form to the Office of Graduate Studies.
Course by Application
This is to register for a course that is not being offered on the current semester schedule.
Form Special Problems Course
This is to apply for a 800 or 801 course.
Form Master's Thesis
Required if enrolling for a Master's Thesis of more than 5 credits.
Form Thesis, Dissertation, Project Specifications
Defense Announcement must be submitted to the Office of Graduate Studies no later than 1-1/2 weeks prior to the scheduled defense.

Doctoral Program Forms
Doctoral Language Examination Report
Used by the department to report foreign language examination results to the Office of Graduate Studies. Foreign language requirement varies by department.
Form Doctoral Qualifying Examination Report
Used by the department to report results of doctoral qualifying examination to the Office of Graduate Studies.
Form Admission to Doctoral Candidacy
Used by the academic department to notify the Office of Graduate Studies that a student is a Candidate for the Ph.D. degree. This is a summary of the above 2 forms.
Link Doctoral Committee Nomination
Used by the student to inform the Office of Graduate Studies of the committee members for the dissertation.
Form Doctoral Activity Report
Used by the student to inform the Office of Graduate Studies of the research status. This form is required at the end of each semester in order to get an
IP grade
Form Doctoral Defense Report
Used by the dissertation committee and department to report the results of the dissertation defense to the Office of Graduate Studies.
Form Thesis, Dissertation, Project Specifications
All theses, dissertations and projects must be submitted to the S.C. Williams Library for archival purposes. The specifications also include the format for the Defense Announcement, which must be submitted to the Office of Graduate Studies no later than 1-1/2 weeks prior to the scheduled defense.

Application for Candidacy & other Graduation Forms
An AC must be submitted to the Office of the Registrar prior to February 1, for December completion, or April 1, for May completion. The AC must have the signature of your advisor. The AC will supersede previous Study Plans.

Letter of Completion/Enrollment Status Request
Use this form if you need certification of your enrollment status or have completed but have yet to graduate.
Form Request for Leave of Absence/Withdrawal
If you need to skip a semester or planning to discontinue your studies here at Stevens, use this form.
Form Program Add/Change
If have been admitted and wish to change your program, or add another program submit this form to the Office of Graduate Admissions. Your application will be forwarded to the new department.
Form Change of Name and/or Address
Use this form to inform the Registrar of a name change and or change of address. A name change requires supporting documentation (e.g., court documents, marriage license, etc.).
Form Non-Workstudy Payroll Notice
Used by the department to charge compensation for Non-Federal Work Study eligible students to departmental budgets.
Form Remediation Action Plan
Remediation Action Plan -Students on academic probation use this form to indicate the steps necessary to return to satisfactory academic standing. The form needs to be signed by the student's academic advisor and submitted to the Registrar's office before enrolling in future semesters.
Form Stanley Fellowship Application
Nominations are open for the Stanley Fellowship. The fellowship is usually awarded on an academic-year basis and includes both tuition and stipend. Nominees must be full-time, United States citizens pursuing a Doctorate or Master's degree and maintaining a 3.5 grade point average. Preference is given to female candidates and students in a Ph.D. program, but this does not exclude male or Master's candidates. For more information, please visit our

Form Please complete the application form (available from the Office of Graduate Admissions and as an Adobe Acrobat file) and submit it to your department director. The department director will recommend qualified students to the Stanley Fellowship committee for consideration.
