Enclosed are many of the forms used by the Registrars Office to conduct various business matters such as enrollment, transcript requests, study plans, etc.

This form is used by students to change their enrollment after prior submission of an Enrollment Form for a semester. Drops during the Add/Drop period at the beginning of a term will result in a removal of the course from the record. A Drop after the end of the Add/Drop period will result in a grade of W being recorded. One page.
This form is used by students to take a course with a Pass-Fail grade. Note that courses taken with a Pass-Fail grade cannot be used towards degree requirements (except PE courses towards the Bachelor's degree). A separate Enrollment or Add/Drop form is not required. One Page.
This form is used to enroll in a Special Problems (800/Master's, 801/Ph.D. or 802/Degree of Engineer, where applicable) course. A separate Enrollment or Add/Drop form is not required. One Page.
This form is used to take a course that is not being currently offered with the permission of the faculty member, department head and dean. A separate Enrollment or Add/Drop form is not required. One Page.
This form is used by undergraduates who wish to "test out" of a course with the permission of the faculty member, advisor and dean. Up to one course per semester may be taken by a Sophomore, Junior or Senior, with suitable academic standing, as a Course by Examination. A separate Enrollment or Add/Drop form is not required. One Page.
This form is required of all Stevens Undergraduate students taking all 600 level courses and above in SOE and SASLA and ALL graduate courses in SOTM. Undergraduates taking such courses are subject to graduate academic review. A 3.0 GPA must be maintained. A student receiving an F, 2 or more C’s or a GPA below 3.0 in graduate courses is no longer eligible to take graduate courses. A separate Enrollment or Add/Drop form is not required. One Page.
This form is used by Graduate students planning to skip a semester or discontinue studies at Stevens. Undergraduate Students must contact the Dean of Undergraduate Academics' Office in order to request a Leave of Absense or Withdraw. A separate Add/Drop form is required. One Page.
This form is used to pay tuition in installments and is available for the current term for students who are in good credit standing with Stevens. One Page.
This form is used by a student to authorize the Registrar's Office to send their employer grade verification information in order to receive tuition reimbursement from their employer after the end of the semester. One Page.
All approved Study Plans and Application for Candidacy forms are to be submitted to the Registrar's office except the graduate Special Student Study Plan, which must be submitted to the Graduate Studies office.
This form is used by Science majors (other than Chemical Biology, Computer Science, Physics or Business and Technology) entering Stevens 2002 Fall for the BS degree. Two pages.
This form is used by Science majors (other than Chemical Biology, Computer Science, Physics or Business and Technology) entering Stevens 2003 Fall or later for the BS degree. Two pages.
This generic form is used for Master of Engineering, Master of Science, Executive Master of Technology Management degrees and Master of Business Administration in Technology Management degrees. One page.

Used by the department to report foreign language examination results to the Office of Graduate Studies. Foreign language requirement varies by department.
Used by the department to report results of doctoral qualifying examination to the Office of Graduate Studies.
Admission to Doctoral Candidacy (Temporarily unavailable)
Used by the academic department to notify the Office of Graduate Studies that a student is a Candidate for the Ph.D. degree. This is a summary of the above 2 forms.
Used by the student to inform the Office of Graduate Studies of the research status. This form is required at the end of each semester in order to get an IP grade.
This form is used to update a student’s online Degree Audit study plan with substitutions, course waivers, or advanced placement credit. It is to be completed by a faculty member.
This form is used to inform the Registrars Office of a legal change in name, ID number (to a valid Social Security Number only) and/or change in permanent or billing address. This form may require separate legal paperwork for validation. Name changes will affect how your name appears on Stevens official documents such as your Transcript, Diploma, Certificate, etc. Addresses can be updated on-line via Web for Students. One Page.
This form is used to request a Letter of Enrollment Status or Letter of Completion. A Letter of Completion can be prepared only after an Approved Application for Candidacy has been submitted to the Office of The Registrar. One Page.
This form is used to request copies (either official or unofficial/student copies) of your transcript. Unofficial Transcripts can be obtained via Web for Students. obtained One page.
This form is used to request that a grading for an enrolled course be changed from the standard letter grade to Pass/Fail. Courses taken with Pass/Fail grades (except PE courses) cannot be used towards degree requirements. One Page.
This form is used to petition the Registrars Office for the grade of INComplete or ABSent and must have the signatures of faculty, department head and dean. A petition for an INC must be filed before the last day of class of a given semester. Two pages.
Graduate Students on academic probation use this form to indicate the steps necessry to return to satisfactory acadmic standing. The advisor's signature is required. A revise Study Plan need to be submitted with this form to the Office of The Registrar. One Page.
Nominations are open for the Stanley Fellowship. The fellowship is usually awarded on an academic-year basis and includes both tuition and stipend. Nominees must be full-time, United States citizens pursuing a Doctorate or Master's degree and maintaining a 3.5 grade point average. Preference is given to female candidates and students in a Ph.D. program, but this does not exclude male or Master's candidates.
This page, maintained by the S.C. Williams Library staff, contains information regarding the format specifications for (Master's) thesis, (Doctoral) dissertation and (Degree of Engineer) projects. All theses, dissertations and projects must be submitted to the S.C. Williams Library for archival purposes. The specifications also includes the format for the Defense Announcement, which must be submitted to the Office of Graduate Studies no later than 1-1/2 weeks prior to the scheduled defense.
