Undergraduate Permission to Take Graduate Courses (16 Kb PDF)

This form is required of Stevens Undergraduate students taking all 600 level courses and above in the School of Engineering and Science, School of Systems and Enterprises, College of Arts and Letters, and all courses in the School of Technology Management.

Permission To Take A Graduate Course Under The Pass-Fail Grading System (64 Kb PDF)

This form is required before you can take a course at Stevens with a Pass/Fail grade.

Incomplete/Absence Petition (64 Kb PDF)

If you cannot complete the requirements for a course and/or are absent for an final examination, you can have an extension of time granted to complete the requirements for the courses. A grade of INC or ABS will be recorded while requirements are being met.

Transfer Credit (65 Kb PDF)

Official transcripts must be submitted to the Office of the Registrar by the institution from which you are transferring credits.

Grade Change

If you suspect that a grade has been incorrectly reported, see the instructor of the course and he will submit this form to the Office of Graduate Academics.

This is to register for a course that is not being offered on the current semester schedule.

Special Problems Course (63 Kb PDF)

This is to apply for a 800 or 801 course.

Master's Thesis (30 Kb PDF)

Required if enrolling for a Master's Thesis of more than 5 credits.

960 Form

Doctoral Language Examination Report (19.5 Kb PDF)

Used by the department to report foreign language examination results. Foreign language requirement varies by department.

Doctoral Qualifying Examination Report (27.0 Kb PDF)

Used by the department to report results of doctoral qualifying examination to the Office of the Registrar.

Admission to Doctoral Candidacy (temporarily unavailable)

Used by the academic department to notify the Office of the Registrar that a student is a candidate for the Ph.D. degree. This is a summary of the above 2 forms.

Doctoral Committee Nomination (23.5 Kb PDF)

Used by the student to inform the Office of the Registrar of the committee members for the dissertation.

Doctoral Activity Report (56 Kb PDF)

Used by the student to inform the Office of the Registrar of the research status. This form is required at the end of each semester in order to get an IP grade.

Doctoral Defense Report (24.9 Kb PDF)

Used by the dissertation committee and department to report the results of the dissertation defense to the Office of the Registrar.

Applications for Candidacy

An Application for Candidacy must be submitted to the Office of the Registrar prior to February 1, for December completion, or April 1, for May completion. The AC must have the signature of your advisor. The Application for Candidacy will supersede previous Study Plans.

Use this form to inform the Registrar of a name change and or change of address. A name change requires supporting documentation (e.g., court documents, marriage license, etc.).

Non-Workstudy Payroll Notice (70 Kb PDF)

Used by the department to charge compensation for Non-Federal Work Study eligible students to departmental budgets.

Remediation Action Plan (61 kB PDF)

Students on academic probation should use the Remediation Action Plan form to indicate the steps necessary to return to satisfactory academic standing. The form needs to be signed by the student's academic advisor and submitted to the Registrar's office before enrolling in future semesters.

Stanley Fellowship Application (86.4 kB PDF)

Please complete the Stanley Fellowship application form (available from the Office of Graduate Admissions and as an Adobe Acrobat file) and submit it to your department director. The department director will recommend qualified students to the Stanley Fellowship committee for consideration.
